Identification
Adults can reach 16 cm in length. Small triangular head, large eyes, slightly humped forehead. Coloration yellow with 5 broad slightly curving horizontal black bands. Bottom band is broken into oblique dashes on belly and anal fin.

Range and habitat
The species occurs in the western Pacific ocean, from Japan and China to New Caledonia. Also found near Hawaii. Lives in coral and rocky reefs in lagoons and coastal areas, to 140 meters deep.
